Occasionally patients do not have access to a computer or are otherwise unable to fill out questionnaires online. If you'd like to provide a paper copy of the forms for them to complete you can actually generate a PDF copy of your questionnaires by printing them as PDFs from the patient portal.
To print questionnaires as PDF, start by logging into your patient portal. From the sidebar, click Questionnaires.
Now, select which questionnaire you would like to print by selecting the green Start button on the lefthand side of the Document.
In your browser, either type Ctrl + P (⌘ + P on macOS) or go to the menu in the top left corner and select Print (this looks like three verticle dots on Google Chrome or three horizontal lines on Mozilla Firefox).
