Many - but not all - clinics that use Cerbo software for their Patient Portal allow new patients to register online. For clinics that do provide this option, it may be available on their Patient Portal login page, or they may make it available by link only. For more information about the online registration options and process for a specific clinic that you are interested in, please reach out to the clinic directly.


Generally, the online registration form will look something like this:


You will need to enter all of the required information (fields marked with an asterisk), as well as other requested information as desired and available. Note for multiple family members in the same practice - multiple people in the same clinic's system may share one email address. Each user will need to have a unique username for accessing the Patient Portal. To avoid confusion, we recommend that family members sharing one email address use unique, identifiable usernames for each family member and do not use the shared email address as the username for any family member.


When you submit the registration form, an automatic confirmation email is sent to the email address that you provided. You must click the link in that email to finalize your registration and activate your Patient Portal access. If you do not receive the confirmation email within a few minutes, please check your spam/ junk mail folder and/ or search for an email from do-not-reply@md-hq.com.


Once you confirm your email address, you can then login to the Portal using the username and the password that you created.


Issues with Registration: Your Account Already Exists

If you try to register as a new patient the system will check to see if your provider has already set up an account for you. The system will look at the first three letters of your first name, your full last name, and your date of birth. If there is anyone already registered with matching details you'll see a message that looks like this:


"According to our records, a patient with your name and date of birth are already registered with our practice. It is possible that the patient we have on file is a different person, but to protect your privacy we ask that you register by phone or in person. Otherwise, if you've just forgotten your password you can try resetting it using the reset password option on the login page (though you'll need to remember the email address you have on file with us."


If that's the case, you can try going through the password-reset process to see if you're able to gain access, otherwise you'll need to contact your provider's office and ask them to invite you to the portal. There are rare situations where families with twin children that have similar first names could get accidentally caught by this duplication check. If that happens, please contact support@cer.bo and let us know which practice you're registering with and we'll assist with getting the second account set up.
